A Care Team Member will schedule a time to visit your home. We always encourage your family, friends, and caregivers to join us to ensure everyone understands the program. During the visit, we’ll gather all of the information we'll need to place you into our program.
We'll also use this time to answer any questions you may have.
All of your information will be sent directly and securely to the pharmacy for a complete and comprehensive medication review.
With the help of our innovative clinical pharmacy systems, our top-notch pharmacists will make sure your prescribed and over-the-counter medications, along with your vitamins, are free of negative drug interactions and that the doses are accurate. If a problem is found, we'll work with your doctors and find a solution.
Finally, you can sit back and relax. Each month you'll receive a new my-medPak box. Use the medPaks as prescribed, and all will be well with your medications!
